How this website handles the information you share with it.

A note on scope. This policy covers the website. Protected health information created once you become a client is governed separately by our HIPAA Notice of Privacy Practices, which you will receive and sign at intake.

Information we collect

When you submit the appointment request form, we collect the information you choose to enter: your name, email address, phone number, contact preferences, who therapy is for, the concerns you select, any message you write, your payment method and insurance plan, format and clinician preferences, and how you heard about us.

Our website host records standard technical information such as IP address, browser type and pages visited, for security and basic traffic analysis.

How we use it

To respond to your request and schedule an appointment
To determine whether we accept your insurance and estimate your cost
To match you with the clinician best suited to your needs
To meet our legal, professional and record-keeping obligations

We do not sell your information. We do not share it with advertisers. We do not use it for marketing unrelated to your request.

The form is not a secure medical record

Email and standard web forms are not encrypted end to end. Please share only what you are comfortable sending by email, and keep detailed clinical or diagnostic information for your session, where it is protected appropriately.

Service providers

We use third parties to run this practice, including our website host and form processor, and Headway for insurance verification and billing. Each is bound to handle information only as needed to provide their service. Once you become a client, your protected health information is handled under HIPAA.

Retention

Form submissions from people who do not become clients are retained only as long as needed to respond, then deleted. Client records are retained for the period required by Wisconsin law and applicable professional standards.

Your choices

Children

This website is intended for adults. We do not knowingly collect information directly from children. Requests concerning a minor should be submitted by a parent or legal guardian.
